Plaza Healthcare

Plaza Healthcare has a Case Management team that works with our patients and their insurance plans to coordinate both their care needs. Every patient at Plaza Healthcare has a Case Manager assigned to them. Our Case Managers work to obtain authorization for our patients care and assure that needed drugs and equipment are acquired. The Case Manager acts as a liaison between the patients health plan and Plaza Healthcare. Case managers will also work with patients and families to develop and carry our a discharge plan.

Guests are welcome to visit patients at Plaza Healthcare daily from 8:00am to 8:00pm. All guests must register with the receptionist and obtain a Guest Badge. Anyone wishing to visit during non-visiting hours must first obtain approval from our Executive Director.
Meal service is available to all guests visiting patients at Plaza. Guests may either pre-purchase a patient meal tray or purchase a meal from the diner. Guests are welcome to dine with the patients either in their room, the diner or in the dining room (must first notify facility if you wish to dine in the dining room)
There are numerous activities available to our patients. Guests are welcome to join our patients during these activities.
