Our Social Worker team works to help meet the psychosocial needs of all our patients. By collaborating with the patient and family, our team can assess the needs of our patients whether it be setting up community services or applying for financial assistance. Each patient is assigned a Social Worker to assess and address their needs.
Plaza Healthcare has a Case Management team that works with our patients and their insurance plans to coordinate their care needs. Every patient at Plaza Healthcare has a Case Manager assigned to them. Our Case Managers work to obtain authorization for our patients care and assure that needed medications and equipment are acquired. The Case Manager acts as a liaison between the patient’s health plan and Plaza Healthcare. Case managers will also work with patients and families to develop and carry out a discharge plan.
