HOSPICE

While it is the goal of Plaza Healthcare for all patients to eventually go home, we understand this is not a possibility in all cases. In some instances, end of life protocols are requested.

Plaza Healthcare has a partnership with Americare Hospice & Palliative Care and uses them exclusively for end of life care. Americare has been delivering hospice care since 2005 uniquely owned and locally operated by a family of Nurse Practitioners and Registered Nurses.

Once the hospice decision has been made, and/or a terminal wean is initiated, know the Plaza team is well trained and will make this transition private, compassionate, and comforting for the patient and ones.
