Plaza Healthcare has an established team of Social Workers dedicated to advocating for patients and their families. Upon admission, your assigned Social Worker will meet with you and/or your family to complete a social history and psychosocial intake assessment.

The Social Worker's focus is on addressing the social and emotional impact of physical or mental impairment, enhancing social functioning, and promoting overall wellness. As part of your multidisciplinary care team, your Social Worker will involve you and your family in ongoing care planning as much as possible.

Plaza Healthcare's Social Worker's are available to provide support through resources as necessary such as Counseling, Psychiatry and Psychology services. They can also assist in setting up in house vendor services for audiology, optometry, and dental care. As well as continuously mediating and advocating for the patient.

The Social Worker in cooperation with the patient’s physician’s, the care team, and family are dedicated to meeting the needs of the patient. In all cases, the Social Worker will build upon existing strengths, enhance adaptations, and modify risks to enable each patient to function at the highest possible level of social and emotional wellness.
