Peritoneal Dialysis
Plaza Healthcare is the only facility to offer a staff-assisted peritoneal dialysis program. With DaVita oversight, under the direction of the Nephrologist, our specially trained RN’s will assist each patient with the peritoneal dialysis treatment. Plaza Healthcare provides the cycler, solutions, and any other necessary supplies to safely administer the treatment in the comfort of the patient’s room.
Diet
Patients with compromised kidney function must adhere to a renal diet to cut down on the amount of waste in their blood. At Plaza Healthcare, the Dietary Services Department recognizes the importance of monitoring a person’s intake of sodium, phosphorus, potassium, protein and fluids. The Dietary Services Department has developed a standard renal diet for dialysis patients. Every person’s body is different, and therefore Plaza’s Registered Dietician will adjust the standard diet to tailor it specifically to each patient’s need. In addition, the renal diet is available in every consistency from regular, to puree, to those requiring nutrition via gastrostomy tube (G-tube).
Transition of Care
For peritoneal patients who have recovered to the point they can discharge, Plaza Healthcare’s Case Management team will work with Home Health and transfusion agencies to ensure the patient has the equipment and supplies necessary to make a safe transition home. Nonetheless, the option to stay long term is always an option.
