DR. OMID DILMAGHANIAN
Medical Director
Dr. Omid Dilmaghanian, otherwise referred to as Dr. D, is the Medical Director at Plaza Healthcare. As the Medical Director, Dr. D is responsible for overseeing the physician-related services in caring for critically ill patients. He actively participates in the Quality Assurance and Performance Improvement, Infection Control, and Interdisciplinary Team (IDT) committees and is very involved in staff education and preadmission evaluations. Dr. D plays a major role in customer service and is influential in the marketing efforts of the facility. He was named the preferred network physician provider for Optum Care of Arizona, Mercy Care, Caremore, and Arizona Care Network (ACN). Dr. D has a doctorate in Medicine from the University of Wyoming: College of Health Sciences, a masters in Health Service Administration at the College of Maine, and is Board Certified in Family Practice. Prior to joining Plaza Healthcare in 2012, Dr. D worked as a staff physician at Cheyenne VA emergency department and Cheyenne Family Medicine Outpatient Clinic for over ten years. He was also a Chief resident at the University of Wyoming: College of Health Sciences. When Dr. D is not working, he enjoys spending time with his wife and 3-year-old daughter. He also enjoys skiing, scuba diving, and traveling.
DAVID STARRETT
Chief Executive Officer
David Starrett is the Chief Executive Officer at Plaza Healthcare. As the CEO, he is responsible for overseeing the overall operation of the facility. He works closely with the management team to assure quality services are provided and regulatory compliance is met. David received his BS in Health Care Administration from Oregon State University in 1997.
Prior to joining Plaza Healthcare in 2007, David worked as a Regional Vice President for Life Care Centers of America. In all, he has 24 years of experience in the health care field. David is also actively involved in the Arizona Health Care Association participating in sub-committees and panel presentations. When David is not working, he enjoys spending time with his wife and three kids, working out, and playing golf.
KIM DELORENZO
Chief Clinical Officer
Kimberly DeLorenzo is the Chief Clinical Officer at Plaza Healthcare. She is responsible for administrative oversight of clinical care operations in line with the facility's strategic mission and vision. As a member of the Executive Team she works closely with operational leaders and managers to ensure excellence in clinical care and regulatory compliance. Kimberly graduated with a Bachelor's in Nursing in 1998 at Waynesburg University located in Pennsylvania.
With twenty five years of experience, Kimberly has a passion and desire to make a difference in healthcare. She has a focus on creating and improving healthcare systems, processes and policies for positive patient outcomes. She has a keen attention to detail and dedication to ensuring the best patient care within a healthcare system.
When Kimberly is not at work, she enjoys spending quality time with her husband and children traveling and exploring new places.
AUDRA BONILLA
Director of Respiratory Services
Audra Bonilla is the Director of Respiratory Therapy at Plaza Healthcare. She is responsible for overseeing all respiratory therapy staff and all respiratory care provided in the facility. Audra received her Associates of Occupational Studies Degree for respiratory therapy in 2001. She was also a member of The Lambda Beta Society of the National Honor Society for the profession of respiratory care during her studies at Apollo College, Mesa Chapter (now Carrington College).
Prior to joining Plaza Healthcare, Audra worked in multiple East valley hospitals to find a good fit and a place to call home. In January 2002 Audra joined Plaza Healthcare as a full time respiratory therapist and never looked back.
Eventually, she worked her way up to Lead therapist in 2015 where she was the go-to RT for leadership within the team of respiratory staff. Audra has spent the last 20 years dedicating her respiratory career to Plaza Healthcare and the patients they so proudly serve.
When Audra is not working, she is enjoying time with her family, training at the gym, riding her mountain bike, kayaking and camping.
ERICA THORNE
Infection Preventionist
Erica Thorne is the Infection Preventionist at Plaza Healthcare. She is responsible for the management and surveillance of healthcare acquired and community acquired infections within Plaza Healthcare. She works closely with the Clinical Team to prevent the spread of infection and ensures proper infection prevention measures are followed. Erica completed her Master of Science in Nursing with a focus on Health Policy from Chamberlain University in 2020.
Erica has a variety of experience including adult and pediatric Critical Care, Surgical Oncology, Hospice and multiple Leadership positions throughout her career. She has 8+ years of experience as a Registered Nurse.
When Erica is not working, she likes spending time with her family & pets, camping, fishing and bowling.
YESENIA VIZCARRA
Director of Case Management
Yesenia Vizcarra is the Patient Care Coordinator Director at Plaza Healthcare. She and her department are responsible for the coordination of services for residents while in the facility as well as during discharge. In addition, Yesenia oversees Guest Relations, Transport, and Reception. Yesenia received her Bachelor's in Social Work in 2015 from Arizona State University.
Yesenia joined Plaza Healthcare as the Director of Social Services but has expanded her role with the oversight of multiple departments. Her passion to help others providing empathy and comfort to our most vulnerable residents is the motivation for her proven success.
When not working, Yesenia enjoys spending time with her husband, 2 daughters and son. She is an avid cook and loves to travel with her family, especially to their most favorite place, Cancun.
TAMMY EVORY
Tammy Evory is the Director of Mimosa Springs at Plaza Healthcare. She is responsible for managing the operations of our memory care community of 42 residents. She is also involved in keeping Mimosa Springs a thriving community by building relationships outside the community, training a high-quality team. Tammy attended the nursing program at GateWay Community College where she discovered her passion to help others.
Prior to joining Plaza Healthcare, Tammy worked for some of the largest senior living companies in the United States as Director of Assisted living for over 12 years. Prior to that she worked as a CNA at Banner University Medical Center on the orthopedic and neurology floor.
When Tammy is not working, she is enjoying time with her family, playing with her dog Oozy, working out and taking short trips on the weekends.
JESSICA RIVERA
Director of Business Development
Jessica Rivera is the Director of Business Development. Jessica received her BS in Nutrition & Sport Science from the University of Idaho in 2008. In 2021 she received her Nursing home Administrators License.
Prior to joining Plaza Healthcare in 2016, Jessica worked as a Rehab Technician at Barrows Neurological Institute. She has held multiple positions at Plaza as the RNA Coordinator, Assistant Administrator and most recently the Director of Business Development.
When Jessica is not working, she enjoys spending time with her husband and two kids, working out, hiking, and watching the Cardinals play football.
