Plaza Healthcare employs a team of Social Workers whose primary mandate is to advocate on behalf of patients and their families. Upon admission, your Social Worker who has been assigned to you for the duration of your stay will meet you and/or your family to complete a social history and psychosocial assessment.

The Social Worker’s focus may be on the social and emotional impact of physical and mental impairment, preservation and enhancement of physical and social functioning, and/or the promotion of physical and mental health. In all cases, the Social Worker will build upon existing strengths, enhance adaptations, and modify risks to enable each patient to function at the highest possible level of social and emotional wellness.

As a member of the multidisciplinary team, the Social Worker will participate in each patient’s ongoing care planning involving the patient and family as much as possible.

Plaza Healthcare’s Social Workers are available to provide support through counseling, mediation, or advocacy to a patient and family member who are struggling with placement. In addition, Social Workers can help families redefine their role and relationship, provide education to patients and family members regarding advance care directives and health care decision making, and assist in mediation and conflict resolution should issues arise.

For those patients requiring more formalized counseling and treatment, Plaza Healthcare has access to Psychiatry and Psychology services that come to the facility on a weekly basis. The Social Worker in cooperation with the patient’s physician’s and the care team will coordinate those services for patients as needed.
