Plaza Healthcare through its partnership with DaVita, offers a wide range of dialysis services to meet the needs of all care requirements for our patients. DaVita is a nationally recognized leader in the dialysis industry, who cares for over 225,000 patients nationwide including over 25,000 in centers such as Plaza Healthcare.

A continuum of care is paramount in delivering quality care which is why the three largest Nephrology groups utilize Plaza Healthcare for their patients. Southwest Kidney, AKDHC, and Desert Kidney all maintain privileges and round at least weekly on their patients.
Dr. Shefeli Gupta, Board Certified in Nephrology, is the Nephrology Medical Director for Plaza Healthcare who has over 20 years of medical experience. In addition, Plaza Healthcare has a dedicated Dialysis Nurse who communicates daily with the Nephrologists and/or Dr. Gupta regarding resident care needs. The Dialysis Nurse also monitors patient lab results, coordinates transfusions and rounds with each Nephrologist when they come to Plaza Healthcare.

Plaza Healthcare’s expertise extends to patients who have the following;
• Acute Renal Failure (ARF)
• Chronic Kidney Disease (CKD, Stages 1-5)
• End Stage Renal Disease (ESRD)
• Renal insufficiency
• Renal Transplant
• Kidney Cancer

In-House Hemodialysis Center
Plaza Healthcare offers a comfortable setting for those patients who cannot utilize outpatient dialysis centers due to their medical and or physical condition. Plaza’s Hemodialysis Center houses five individual self-contained bays for privacy of care. Patients are brought to the Dialysis Center in their own bed, thus providing an easy transition to and from their treatments. Patients can enjoy watching TV on their personal ceiling-mounted television or request a variety of reading or listening material from our Activity Department to help pass the time during treatment.

Bedside Hemodialysis
For those patients unable to take advantage of the Hemodialysis Center, Plaza Healthcare offers bedside hemodialysis. With multiple portable machines stored on site, Plaza can easily accommodate the needs of those patients with conditions warranting in-room or bedside hemodialysis.

Peritoneal Dialysis
Plaza Healthcare is the only facility to offer a staff-assisted peritoneal dialysis program. With DaVita oversight, under the direction of the Nephrologist, our specially trained RN’s will assist each patient with the peritoneal dialysis treatment. Plaza Healthcare provides the cycler, solutions, and any other necessary supplies to safely administer the treatment in the comfort of the patient’s room.

Blood Transfusion
At times, it is necessary for patients on dialysis to be scheduled for blood transfusions. Plaza Healthcare has the ability to perform blood transfusions in-house to reduce re-hospitalization and to maintain continuum of care. Due to insurance plan restrictions, some patients will go out for the procedure. Plaza Healthcare has an established relationship with HonorHealth Osborn and in cases requiring it; transfusions are performed in their Emergency Department. 

Personalized Schedule
As part of the continuum of care mentioned earlier, Plaza Healthcare will do its best to maintain the schedule of dialysis treatments established prior to arriving. With a minimum of two dialysis runs per day, six days a week, as well as our own nurses administering the peritoneal dialysis treatments, Plaza Healthcare has the flexibility and resources to accommodate most schedules.

Diet
Patients with compromised kidney function must adhere to a renal diet to cut down on the amount of waste in their blood. At Plaza Healthcare, the Dietary Services Department recognizes the importance of monitoring a person’s intake of sodium, phosphorus, potassium, protein and fluids. The Dietary Services Department has developed a standard renal diet for dialysis patients. Every person’s body is different, and therefore Plaza’s Registered Dietician will adjust the standard diet to tailor it specifically to each patient’s need. In addition, the renal diet is available in every consistency from regular, to puree, to those requiring nutrition via gastrostomy tube (G-tube).

Transition of Care
While some dialysis patients at Plaza Healthcare eventually resolve their compromised kidney function, most patients will need to continue treatment beyond their initial stay. Plaza Healthcare’s Case Management department will work with the patient and their family to transition care to an outpatient clinic in the area the patient resides or will be residing. There are certain criteria that must be met for a patient to be able to have outpatient treatments and Plaza’s transition team will prepare the individual with test runs to ensure their success within the community.

For those patients with established chairs within the community, Plaza Healthcare will work to re-initiate that service for the patient. For those requiring a new chair, Plaza Healthcare has relationships with outpatient clinics across the entire State of Arizona as well as surrounding States for those patient’s living in New Mexico, Nevada, Utah, Colorado or California. Whether it is a DaVita or Fresenius outpatient clinic, the Case Management team will make arrangements for an outpatient chair as close to where the patient lives as possible.

For peritoneal patients who have recovered to the point they can discharge, Plaza Healthcare’s Case Management team will work with Home Health and transfusion agencies to ensure the patient has the equipment and supplies necessary to make a safe transition home.
