Plaza Healthcare employs a team of Case Managers whose primary role is to act as a liaison on behalf of the patient between the insurance company and Plaza Healthcare. This responsibility of coordinating care involves ensuring the proper authorizations are acquired for the services and equipment a patient needs.

Each patient is assigned a Case Manager and that person will update the insurance company as required on a patient’s current status and/or progress. The Case Manager will work closely with the patient or responsible party to regularly update them as to the extent of coverage, and recommend next steps to take if an extended stay is required or coverage is being terminated by the insurance company.
Your Case Manager will also play a key role in applying for financial assistance if required. Each Case Manager is trained and has years of experience in the ALTCS application and are available to assist patients and families through the process.

Discharge
At Plaza Healthcare, discharge planning begins upon admission by understanding each patient’s goal of placement once recovered. Throughout a patient’s stay, this may change based on progress, or other factors that may prohibit or enhance the initial goal.
As a member of the multidisciplinary team, the Case Manger will work with each team member, attending physicians, patient and family to recommend a placement most suitable for a safe and successful transition.

This may be returning home, converting to long term care within Plaza Healthcare, or to another facility such as acute rehab or an assisted living community. The Case Manager regardless of scenario will work with the patient and family and the insurance company to arrange any community services and/or equipment necessary for the transition.
